Creating Intake Forms

There are multiple ways of creating Intake Forms in Abacus. Choose the method that is most efficient for you.

From an Intake Form template

It is always easier to start with an existing Intake Form than it is to start from scratch. Abacus provides several templates for this purpose.

  1. Select Matters > Intake Forms > Intake Form Manager. The Intake Form Manager window appears.

  2. Click Add and click From Template. The Intake Form Templates window appears.

  3. Highlight the template you want to use and click Select.

  4. The Web and Intake Form Designer window appears. At this point, you can edit the Intake Form and save it using a different name.

From the standard default Intake Form

This method provides a basic starting point.

  1. Select Matters > Intake Forms > Intake Form Manager. The Intake Form Manager window appears.

  2. Click Add and click Default Sample Form. A window appears prompting you to enter a new name for the Intake Form.

  3. Enter the new name and click Done. The new Intake Form is created using the default sample format, and it is added to the list of Intake Forms on the Intake Form Manager window. Edit the Intake Form as necessary.

From scratch

This method provides a blank starting point.

  1. Select Matters > Intake Forms > Intake Form Manager. The Intake Form Manager window appears.

  2. Click Add and click From Scratch. A window appears prompting you to enter a new name for the intake form.

  3. Enter the new name and click Done.

  4. The Web and Intake Form Designer window appears. Complete the designer.

Through cloning

Use this method to create a similar Intake Form.

  1. Select Matters > Intake Forms > Intake Form Manager. The Intake Form Manager window appears.

  2. Highlight the form you want to clone and click Clone. A window appears prompting you to enter a new name for the intake form.

  3. Enter the new name and click Done.

  4. The Web and Intake Form Designer window appears. Complete the designer.
